Documents Needed for Enrollment
- Registration Form
- Completed Registration Form (If you need help, one of our enrollment specialists will be happy to assist you over the phone at 602-334-4104.)
- Proof of Residency Form and Supporting Documents (Be sure the documents submitted match what is checked on the residency form.)
Additional Documents for Attendance
- Birth Certificate or other reliable proof of the student’s age and identity to be provided 30 days within the date of enrollment.
- Official Transcript from the last school attended
- Completed Records Request form
- Official Withdrawal from the last school attended
- Immunization Record or completed exemption form
- Mc-Kinney-Vento Forms (only for homeless or unaccompanied youth)
